Referrals, Nutrition for a Healthy Life

Information for GPs, specialists, and NDIS support coordinators referring patients for dietetic services.

GP & Specialist Referrals

We accept referrals from GPs and specialists for patients requiring dietetic management of chronic conditions, complex dietary needs, or acute nutrition support.

Medicare Chronic Disease Management (CDM) Plans

Patients with an eligible chronic condition may be entitled to up to 5 Medicare-subsidised allied health visits per calendar year with a GP Mental Health Treatment Plan or Chronic Disease Management Plan (formerly EPC). Ask your GP about a referral to a dietitian under this scheme.

DVA Referrals

We accept Department of Veterans' Affairs (DVA) referrals. Gold and White Card holders may be eligible for dietitian services.

How to Refer

GPs and specialists can refer directly by:

  • Completing an Allied Health Referral form (standard format accepted)
  • Faxing or emailing to our practice
  • Patients can also self-refer, no GP referral is required for private consultations

NDIS Support Coordinator Referrals

We work with NDIS participants who are plan-managed or self-managed. Support coordinators can refer participants for dietetic assessment and ongoing nutrition support.

What We Provide for NDIS Participants

  • Initial nutrition assessment and goal setting
  • Individualised meal plans
  • Texture-modified diet and dysphagia support
  • Carer and family education
  • Tube feeding assessment and management
  • NDIS progress reports (service agreement required)
