We accept referrals for patients requiring dietetic management of chronic conditions. Patients with an eligible chronic condition may access up to 5 Medicare-subsidised visits per year with a GP Chronic Disease Management (CDM) Plan.
We also accept DVA referrals (Gold and White Card holders). Patients can also self-refer. No GP referral is required for private consultations.
We work with plan-managed and self-managed NDIS participants. Support coordinators can refer participants directly for nutrition assessment and ongoing support.
